Virtual MIDI patch bay...

Many HWs like V-synth and Fantom have USB-MIDI. I would like a program (a Virtual MIDI patch bay) that can route, merge, split etc those virtual MIDI (USB) conections to and from each other. You can do something like it with the big dragon DAWs like Sonar but I'm looking for a small, self contained program.

Is there something like that out there?
